What you will need to register for Health Insurance Checklist

This includes any offers of health coverage, employer contact information, W-2 forms, pay stubs, Social Security payments, retirement income, investment income, or other taxable income.

Health coverage information

If anyone in your household has a health plan through their employer or through the Federal government, you will need their policy numbers.

Current doctors and medications

Having a list of your doctors and medications in front of you will be helpful when comparing different plans.
